Please note the doctors at Cadbury Heath Healthcare will not sign any ‘fit for’ forms. These forms are usually in connection with outdoor pursuits such as skydiving, deep sea diving, marathons outside of the UK.

If you are intending on taking part in an activity that requires a doctors signature please make an appointment with a private GP.
